To start your Employment Insurance (EI) Claim go to the
Service Canada Application for Employment Insurance On-Line Form
If you are not comfortable doing this from home, you can use the computers in our office or go to the Service Ontario Office Downtown at 400 City Hall Sq E.
You'll need between 30 and 60 minutes to complete the on-line form

Information you will need
- Social Insurance Number (SIN). If your SIN begins with a 9, you need to supply proof of your immigration status and work permit.
- Your mother's last name when she was born.
- Postal code of your usual place of residence. Please note that if you do not have a usual place of residence, you must apply in person at your local office.
- Your complete bank information (Branch number, Financial Institution - name and number and Account number), as shown on your cheque or bank statement so your payments can be deposited directly into your bank account.
- Your detailed version of facts if you have quit or have been dismissed from any job in the last 52 weeks.
- The name, address, dates of employment and reason for separation for all your employers in the last 52 weeks.
- The following details about your most recent employment:
- Salary amount before deductions including tips and commissions.
- Salary amount before deductions for your last week of work (from Sunday to your last day of work)
- Amounts received or to be received as:
- Vacation pay
- Severance pay
- Pension
- Pay in lieu of notice
- Other money (you will need to specify)
- Dates for the weeks (Sunday to Saturday) in the last 52 weeks when you did not work, did not receive monies and reasons why.
- Dates and salary before deductions for weeks (Sunday to Saturday) of employment in the last 52 weeks if one of the following situations applies to you:
- you have weeks where your earnings before deductions were less than $225 but more than $0.00 per week,
- You reside in one of the economic regions designated under the “Best 14 weeks” pilot project and you are not applying for fishing benefits.
- If you are applying for parental benefits you will need the SIN of the other parent.
- If you are applying for sickness benefits you will need to provide your doctor's name, address, telephone number and how long your illness is expected to last.
- If you are applying for compassionate care benefits you will need information about the gravely ill family member, such as first and family name, date of birth and residential address.
